Service Description
Over a three month period, the Patient and their Family (if desired) will receive a series of ten 90 minute meetings (approximately 3 meetings per month) where I will support the Patient and their Family to identify, document and implement the Five Domains of Life, which are life fulfillment of wishes in the following priorities: 1. Spiritual 2. Emotional 3. Physical 4. Life-Legacy, and 5. After Death Care
